Improve patient hand-off in the medical surgical unit at St. Francis Medical Center to reduce communication errors, improve patient safety, and health outcomes.

My part is to write an ACTION PLAN: I am only responsible in writing the first half of this part because it is shared between 2 people. Please refer to the GANTT CHART (attached) regarding our action plan.
Define an action plan to improve outcomes related to your identified problem. Include your preferred evidence-based tools, resources, frameworks that seem practical to analyze and plan for improvement. (For example, Data collection/analysis; Root cause analysis, FMEA, Cause/effect analysis; SWOT analysis; SMART objective; PICOTS, concept map, change management methods/theories/interventions, leadership frameworks). Critically reflect on WHAT you want to change, HOW you want to improve, and specific measures of outcomes that indicate continuous improvement/success.
